Join YPAC and the United Way of Greater Chattanooga on July 16, 2011, for a day filled with fun and fundraising as YP's raise funds for the United Way. Form a kickball team with your co-workers or friends and compete for the coveted YPAC Kickball Trophy.

NOTE: Day of registration begins at 9:30 a.m. Game time actually begins at 11 a.m. The final game of the day should begin at 7:30 p.m. and is expected to end by 9 p.m.

A team consists of at least 8 team members with a maximum of 15. Team captains should fill out this form; all participants must additionally fill out a release form at the event.

Registration is $250 per team. If more than ¾ of your team is comprised of YPAC members, then your rate is $175. All forms should be sent to YPAC, PO Box 11451, Chattanooga, TN 37403. Payment can be made via check or money order made payable to YPAC.
